Booking and Cancellation Policy: Although we do not require a valid credit card on file to reserve your reservation, we do require at least 12 hours’ notice to cancel any appointment. Once an appointment is booked, unless we hear from you, we’ll expect (and are excited for) you to be there. Salon D’ Shayn does not currently enforce a cancellation fee, however, serial cancellations will cause you to be refused future services.
Consent for Waxing: I hereby authorize Salon D’ Shayn and it’s employees to perform a waxing procedure and fully understand this procedure has limited applications. I understand that each person’s skin and hair are unique and there is no guarantee that side effects will not occur. I am aware that waxing may cause health risks such as: inflammation, irritation, welts, hives, reddening of the skin, or small breakouts. Salon D’ Shayn proudly carries waxing after care products specifically designed to prevent breakouts, ingrown hairs and other skin irritations, just ask!
By booking an appointing with Salon D’ Shayn, I attest beyond the shadow of a doubt that I will disclose any allergies as well as the use of any and all medications; including but not limited to: antibiotics, steroids, topical acne medicines (Accutane, Differin, Retin A, Prednisone or Corticosteroids or any others). I understand and acknowledge there are risks involved with the waxing procedure, including but not limited to those side effects listed above.
I understand any false or misleading information I have given may lead to undesired results and unnecessary health complications and hereby waive and release Salon D’ Shayn from any and all liability if such results or complications occur for any reason. I further understand my failure to follow professionally recommended post care instructions may also lead to undesired results, complications or effects and hereby waive and release Salon D’ Shayn from any and all liability if such results or complications occur for any reason. In addition, if any complications or concerns do arise, I will bring this to the attention of Salon D’ Shayn within 24 hours of my service.
